Diagnose validation test failure

Explain why an FFv2 validation test pipeline failed and what to do next. Uses backend-authoritative, bounded ADO evidence to classify the failure as product-regression, test-infrastructure, or unknown. Call this before requesting a full-pipeline rerun; only a rerun-eligible result may proceed.
